What to prepare?

Please complete patient intake forms below by either filling them out online or via paper. By completing these forms ahead of time, you will save a significant amount of time during your visit .
If it is not possible to complete these forms in advance, we will have them ready at the testing site.

When to arrive?

Please arrive at the designated parking spaces 5 minutes prior to your scheduled appointment. 

What to bring?

• If you fill out the forms online, please bring your driver’s license and insurance card to the testing site.
• If you fill out the forms via paper, please bring the completed forms along with your driver’s license and insurance card to the testing site.

*We will have the forms at the testing site if you don’t have them ready.
